So it works ok when cold?

That would eliminate the fuel filter or fuel pump, more than likely a vacuum leak at the throttle body, intake manifold or an ancillary hose. Other possible issues would also affect it when cold such as timing chain tensioner or dodgy plugs, blocked injectors. Is the clutch ok not slipping or anything?

Compression in all cylinders ok?

Welcome to the club matey

A theory is that when it get's "Warm" it may be overheating. An engine that is too hot will swell causing components to get sticky. Keep an eye on that temp gauge and it may be a good idea to flush/change the coolant.

This theory comes directly from personal experience. My temp gauge was showing the engine to be at the correct temperature but it was reading wrong due to a big air-lock. Symptoms were lack of performance once engine warmed up. This resulted eventually in engine failure.
